Most women with early-stage breast cancer avoid extensive lymph node removal

Until now, it was unclear to what extent surgeons were following the recommendations of a landmark clinical trial published more than four years ago, known as the American College of Surgeons Oncology Group Z0011, or ACOSOG Z-11, trial. Those researchers reported that most early-stage breast cancer patients with tumor in their sentinel lymph node (the first draining node) who undergo lumpectomy do not benefit from surgical removal of the remaining lymph nodes in the underarm area, called completion axillary lymph node dissection (ALND). That study found no difference in cancer recurrence1 and five-year survival2 between patients who underwent ALND and those who did not but were monitored for recurrences…

Precision medicine for adrenal cancer, study suggests

In a randomized phase 3 trial, adrenal cancer patients receiving the investigational drug linsitinib fared no better than patients receiving a placebo. But the researchers noticed a small subset of patients who had significant response and remained on the drug for an extended time. “While it was only a small subset of patients who responded to linsitinib, this remains very promising in the era of precision medicine,” says co-principle investigator Gary D. …

Hot on the trail of hepatitis-liver cancer connection

For the study, which was published in Nature Communications, the group performed whole genomic sequencing on 30 individual tumors classified as liver cancer displaying a biliary phenotype. This type of cancer originates in the liver, but is different from hepatocellular carcinoma, the dominant form of primary liver cancer, and is generally more aggressive, with poorer prognosis. …

Vaccine therapy for prostate cancer patients with rising PSA examined

As part of a Phase II clinical trial, adult patients with advanced prostate cancer (as evidenced by two rising prostate-specific antigen or PSA values and no visible metastasis) whose cancer is resistant to hormone therapy and had either surgery or radiation were recruited from member institutions in the ECOG-ACRIN Cancer Research Group. …

Experience counts with radiation therapy for head and neck cancer, study shows

Published in the Journal of Clinical Oncology with an accompanying editorial, the study compared survival and other outcomes in 470 patients treated with radiation therapy at 101 treatment centers through a clinical trial held from 2002 to 2005. The trial was sponsored by the National Cancer Institute and organized by the Radiation Therapy Oncology Group (RTOG). The findings indicated that patients treated at the less-experienced centers were more likely to have cancer recurrence (62 percent versus 42 percent at five years) and had poorer overall survival compared with those at the highly-experienced centers (51 percent versus 69 percent five-year survival, respectively)…

Substantial improvement in England, Wales cancer survival over 40 years overshadowed by low survival for brain, stomach, lung, oesophageal, and…

Led by the Cancer Research UK Cancer Survival Group at the London School of Hygiene & Tropical Medicine, London, UK, the researchers analysed survival trends for more than 7 million adults (aged 15 to 99 years) diagnosed with one of 21 common cancers in England and Wales between 1971 and 2011, and followed up to the end of 2012. The 21 cancers clearly divided into three groups with either high, moderate, or low survival…

Nonsmokers in automobiles are exposed to significant secondhand smoke

The nonsmoking passengers showed elevated levels of butadiene, acrylonitrile, benzene, methylating agents and ethylene oxide. This group of toxic chemicals is “thought to be the most important among the thousands in tobacco smoke that cause smoking-related disease,” said senior investigator Neal L. Benowitz, MD, a UCSF professor of medicine and bioengineering and therapeutic sciences and chief of the division of clinical pharmacology at San Francisco General Hospital and Trauma Center. …

Scientists challenge efficacy of folfiri in a colorectal cancer subtype

Conclusions from the analysis, led by Manuel Hidalgo, Director of the Clinical Research Programme at CNIO, are in response to previous work published in the same journal in March, 2014 (Sadanandam et al). By means of analysing gene expression profiles, Sadanandam’s team designed a classification for colon cancer based on five subtypes, associating one of them, the stemlike subtype, with a better drug response to FOLFIRI; the standard treatment for this tumour type. Specifically, in a 21-patient group, 7 were classified as this subtype, 5 of whom (70%) exhibited a positive response to FOLFIRI. The authors of this article therefore concluded that the classification based on gene expression profiles is directly related to the drug response…

Breast cancer recurrence: Study provides insight — ScienceDaily

Around 5,000 cases of DCIS, a condition where cancerous cells are contained within the milk ducts of the breast, are diagnosed each year in the UK, with two thirds diagnosed through breast screening. If left untreated, up to half of DCIS cases could progress into invasive breast cancer, but it is not possible to say which ones, so all women are offered treatment. …

Immune booster combined with checkpoint blocker improves survival in metastatic melanoma

Patients in the clinical trial who got the combined therapies also had fewer serious adverse side effects than those who received only ipilimumab, the researchers report in the Journal of the American Medical Association. The group treated with both ipilimumab and the immune stimulant, called sargramostim, had a one-year survival rate of 68.9 percent vs. 52.9 percent in the ipilimumab-only group. In both groups, however, the median progression-free survival (the length of time before the cancer began to grow) was similar – 3.1 months…
