What you need to know about new PSA screening guidelines

Recently, the American Urological Association (AUA) announced new guidelines for prostate specific antigen (PSA) testing.  These guidelines were designed to help urologists, and ultimately patients, reduce prostate cancer mortality by making informed screening decisions.  These recommendations were based on comprehensive literature reviews and the strength of the existing evidence.   Here is what you need to know: • The AUA recommends against screening in men under age 40.  Such blanket recommendations can increase the risk of younger men being overlooked and potentially resulting in worse disease later in life. Men under 40 need to be educated about prostate cancer and given a clear understanding of their individual risk factors.   Obesity/excess weight, a family history of prostate cancer, and African American race are the driving risk factors of this disease. • The AUA recommends against routine screening in men of average risk aged 40 to 54 years old.  Early diagnosis and treatment are the two most important factors to successfully eliminating prostate cancer. • The AUA strongly recommends careful consideration of the pros and cons of screening for men between the ages of 55 and 69.  The risk of prostate cancer increases significantly by age 65; so it’s wise for men in this age bracket to be screened. These men have the greatest opportunity for early detection and treatment to eliminate their prostate cancer with optimal quality of life. • The AUA suggests screening for prostate cancer every two years rather than annually. PSA is not a perfect test.  Prostate cancer can be an indolent cancer, taking many years to decades before it causes problems or it can behave in a highly aggressive manner. PSA is not able to differentiate these two cases. However, by tracking PSA velocity and density we can more accurately predict one’s risk of cancer. • The AUA recommends against PSA screening in men over age 70 with a life expectancy less than 10-15 years.  Now that such a large percentage of Americans live well into their 80s, prostate cancer screening should be part of overall wellness monitoring for these men. Prostate cancer remains the second leading cause of cancer death in men, killing approximately 34,000 men each year.  Since the widespread adoption of PSA screening in the early 90s, there has been a 39 percent reduction in prostate cancer mortality rates; so there is no doubt that PSA screening is successful – when used correctly.   Speak with your doctor about your individual risk factors for prostate cancer and your treatment goals.  Through comprehensive education about prostate cancer testing, diagnosis, and treatment options, American men can make well-informed decisions about what’s best for them. New cancer tools allow patients to reconsider chemo

After decades of using one-size-fits-all therapies to combat cancer, doctors are using new tools to help decide when their patients can skip chemotherapy or other harsh treatments. An approach to oncology that has been in place for decades is beginning to yield to an arsenal of long-term clinical studies, genetic tests and novel drugs that target cancer cells and their infrastructure. “What is happening is a combination of new technology and more-targeted cancer drugs,” said Dr Sandra Swain, medical director of the Cancer Institute at Washington Hospital Center and president of the American Society of Clinical Oncology (ASCO). “We've tried the approach of big, nonspecific treatments ... We have found that throwing chemo at patients has not (necessarily) cured them.” Traditional chemotherapy drugs work by interfering with the entire body's system of cell replication, causing harsh side effects like fatigue and hair loss. Since the completion of the human genome project in 2003, scientists have made progress in unlocking the genetic basis of a range of diseases, including cancer. That has paved the way for genetic testing as well as drugs that block specific pathways that cancer cells use to grow and reproduce. Such targeted cancer drugs, which sometimes preclude the need for chemotherapy, are being sold by companies ranging from Pfizer Inc, the world's largest drugmaker, to Ariad Pharmaceuticals Inc, which early this year launched its first drug, to treat leukemia. At the same time, large-scale studies that look at whether some types of patients are better off with less treatment are giving doctors more confidence to hold off on using traditional cancer drugs. Laurie Levin, now 64, was successfully treated in her 20s for non-Hodgkin's lymphoma, but faced a dilemma after being diagnosed with breast cancer in 2005 since the earlier radiation and chemotherapy had already raised her risk of developing heart problems or leukemia. A $4,000 genetic test showed that her breast cancer was unlikely to return, providing the confidence to undergo a lumpectomy and avoid chemotherapy. “It was like someone handed me my life back when I got those results,” she said. Use of the Oncotype DX test, which analyzes genes involved in tumor recurrence, has cut the use of chemotherapy in U.S. breast cancer patients by 20 percent over the past eight years, according to its maker, Genomic Health Inc. The company recently launched a similar test designed to measure whether men with prostate cancer need to undergo surgery or radiation. Tests and studies can clarify treatment, but costs remain on the upswing because the newest drugs are very expensive, with monthly price tags often in the thousands of dollars. By 2016 annual global sales of cancer drugs will nearly triple, to $88 billion from a decade earlier, according to IMS Health. 'RIGHT-SIZING TREATMENT' The “less is more” approach to cancer will be one highlight of ASCO's annual meeting in Chicago that begins at the end of this month. On Wednesday, ASCO released thousands of abstracts on new clinical trials of cancer treatments. One large, long-term study found that most men diagnosed with early-stage seminoma, a common type of testicular cancer, did fine with no treatment following surgery to remove the tumors. Cure rates for the disease have always been quite high. Several European countries, including Denmark where the study was conducted, monitor seminoma patients for any relapse before further treatment. In the United States, about half of early-stage patients are still given radiation or chemotherapy, according to ASCO. “Opting for surveillance spares patients, most of whom are young men, from the harmful side effects of chemotherapy and radiation without diminishing their chances for a long and healthy life,” said ASCO's incoming president, Dr Clifford Hudis, in a statement. Physicians say it is difficult to quantify in statistics, but there is growing recognition that less is more in terms of potentially toxic cancer treatments. The approach is especially important for young patients who will have many years ahead of them after beating an initial bout of cancer. “We are right-sizing treatment,” said Dr. James Mohler, chair of the department of urology at Roswell Park Cancer Institute in Buffalo, New York. He pointed to recent national guidelines calling for “active surveillance” of older men diagnosed with slower-growing prostate cancer. A study presented earlier this year at an ASCO meeting in Florida found similar survival rates for men with high-risk prostate cancer who received radiation and either 18 or 36 months of hormone therapy. The findings suggest the therapy, which causes significant side effects, could be given for less than the current standard of 24 to 36 months. Another recent study out of the Duke Cancer Institute in Durham, North Carolina, found that survival odds for women with early-stage breast cancer who underwent breast-preserving surgery such as lumpectomy were as good as, or even better than, the odds for women who had mastectomies. “We are going to see reevaluations of very successful therapies to determine whether or not we can achieve the same results using less treatment,” said Dr Armand Keating, director of the hematology division at the University of Toronto and president of the American Society of Hematology. The first-ever study showing that a type of leukemia could be cured without using chemotherapy was released in December. The Italian-German study found that a combination of a derivative of vitamin A, known as ATRA, and arsenic trioxide, a newer drug, worked as well as ATRA and chemotherapy in patients newly diagnosed with acute promyelocytic leukemia (APL). “APL used to be one of the most dreaded strains of cancer, but with ATRA and chemo the results are very gratifying,” Keating said. “Now we have two agents that are not chemo agents ... That to me is a milestone. I can't see any reason why this wouldn't become the standard of care.” A recent trial conducted in France found that omitting standard chemotherapy, which has been linked to heart damage, from the initial treatment of a type of childhood leukemia did not reduce survival outcomes. “The nice thing is you have omitted a potentially toxic agent that contributes to morbidity and maybe mortality down the road,” Keating said. The priciest therapies are designed to take advantage of genetic mutations associated with cancer cells, some of them found only in a small percentage of patients. A new drug for melanoma, BRAF inhibitor Zelboraf from Roche Holding AG, is designed to work by targeting a specific genetic mutation found in about half of all melanomas. Patients are first tested to see if they have it. Pfizer's lung-cancer drug Xalkori, which targets a mutation in the ALK gene, works in about 4 percent of lung cancer patients. It also has been effective as a treatment for a rare but aggressive type of childhood lymphoma. “We've been really trying for years to be more precise about who needs treatment ... First prospective trial shows molecular profiling timely for tailoring therapy

New approaches give women more options for breast cancer surgery

CHICAGO – & Treating breast cancer almost always involves surgery, and for years the choice was just having the lump or the whole breast removed. Now, new approaches are dramatically changing the way these operations are done, giving women more options, faster treatment, smaller scars, fewer long-term side effects and better cosmetic results. It has led to a new specialty -- “oncoplastic” surgery -- combining oncology, which focuses on cancer treatment, and plastic surgery to restore appearance. “Cosmetics is very important” and can help a woman recover psychologically as well as physically, said Dr. Deanna Attai, a Burbank, Calif., surgeon who is on the board of directors of the American Society of Breast Surgeons. Its annual meeting in Chicago earlier this month featured many of these new approaches. More women are getting chemotherapy or hormone therapy before surgery to shrink large tumors enough to let them have a breast-conserving operation instead of a mastectomy. Fewer lymph nodes are being removed to check for cancer's spread, sparing women painful arm swelling for years afterward. Newer ways to rebuild breasts have made mastectomy a more appealing option for some women. More of them are getting immediate reconstruction with an implant at the same time the cancer is removed rather than several operations that have been standard for many years. Skin and nipples increasingly are being preserved for more natural results. Some doctors are experimenting with operating on breast tumors through incisions in the armpit to avoid breast scars. There's even a “Goldilocks” mastectomy for large-breasted women -- not too much or too little removed, and using excess skin to create a “just right” natural implant. Finally, doctors are testing a way to avoid surgery altogether, destroying small tumors by freezing them with a probe through the skin. “Breast surgery has become more minimalistic,” said Dr. Shawna Willey of Georgetown's Lombardi Comprehensive Cancer Center. “Women have more options. It's much more complex decision-making.” Breast cancer is the most common cancer in women around the world. In the U.S. alone, about 230,000 new cases are diagnosed each year. Most can be treated by just having the lump removed, but that requires radiation for weeks afterward to kill any stray cancer cells in the breast, plus frequent mammograms to watch for a recurrence. Many women don't want the worry or the radiation, and choose mastectomy even though they could have less drastic surgery. Mastectomy rates have been rising. Federal law requires insurers to cover reconstruction for mastectomy patients, and many of the improvements in surgery are aimed at making it less disfiguring. Here are some of the major trends: IMMEDIATE RECONSTRUCTION Doctors used to think it wasn't good to start reconstruction until cancer treatment had ended -- surgery, chemotherapy, radiation. Women would have a mastectomy, which usually involves taking the skin and the nipple along with all the breast tissue, followed by operations months later to rebuild the breast. Reconstruction can use tissue from the back or belly, or an implant. The first operation often is to place a tissue expander, a balloon-like device that's gradually inflated to stretch the remaining skin and make room for the implant. A few months later, a second surgery is done to remove the expander and place the implant. Once that heals, a third operation is done to make a new nipple, followed by tattooing to make an areola, the darkened ring around it. The new trend is immediate reconstruction, with the first steps started at the time of the mastectomy, either to place a tissue expander or an implant. In some cases, the whole thing can be done in one operation. Nationally, about 25 to 30 percent of women get immediate reconstruction. At the Mayo Clinic, about half do, and at Georgetown, it's about 80 percent. SPARING SKIN, NIPPLES Doctors usually take the skin when they do a mastectomy to make sure they leave no cancer behind. But in the last decade they increasingly have left the skin in certain women with favorable tumor characteristics. Attai compares it to removing the inside of an orange while leaving the peel intact. “We have learned over time that you can save skin” in many patients, Willey said. “Every single study has shown that it's safe.” Now they're going the next step: preserving the nipple, which is even more at risk of being involved in cancer than the skin is. Only about 5 percent of women get this now, but eligibility could be expanded if it proves safe. The breast surgery society has a registry on nipple-sparing mastectomies that will track such women for 10 years. “You really have to pick patients carefully,” because no one wants to compromise cancer control for cosmetic reasons, Attai said. “The preliminary data are that nipple-sparing is quite good,” but studies haven't been long enough to know for sure, Willey said. “It makes a huge difference in the cosmetic outcome. That makes the woman's breast recognizable to her.” Dr. Judy Boughey, a breast surgeon at the Mayo Clinic, said the new approach even has swayed patients' treatment choices. “We're seeing women choosing the more invasive surgery, choosing the mastectomy,” because of doctors' willingness to spare skin and nipples, she said. It helped persuade Rose Ragona, a 51-year-old operations supervisor at O'Hare Airport in Chicago. She had both breasts removed on April 19 with the most modern approach: Immediate reconstruction, with preservation of her skin and nipples. “To wake up and just see your breasts there helped me immensely,” she said. She chose to have both breasts removed to avoid radiation and future worry. “I felt it was a safer road to go,” she said. “I can't live the rest of my life in fear. Every time there's a lump I'm going to worry.” FREEZING TUMORS Attai, the California breast surgeon, is one of the researchers in a national study testing cryoablation. The technique uses a probe cooled with liquid nitrogen that turns tumors into ice balls of dead tissue that's gradually absorbed by the body. This has been done since 2004 for benign breast tumors and the clinical trial is aimed at seeing if it's safe for cancer treatment. “The technology is amazing. This is done in the office under local anesthesia, a little skin puncture,” Attai said. In the study, women still have surgery at some point after the freezing treatment to make sure all the cancer is destroyed. Cancer drug prevents build-up of toxic brain protein

They say their study, published online May 10 in Human Molecular Genetics, offers a unique and exciting strategy to treat neurodegenerative diseases that feature abnormal buildup of proteins in Parkinson’s disease, Alzheimer’s disease, amyotrophic lateral sclerosis (ALS), frontotemporal dementia, Huntington disease and Lewy body dementia, among others. "This drug, in very low doses, turns on the garbage disposal machinery inside neurons to clear toxic proteins from the cell. By clearing intracellular proteins, the drug prevents their accumulation in pathological inclusions called Lewy bodies and/or tangles, and also prevents amyloid secretion into the extracellular space between neurons, so proteins do not form toxic clumps or plaques in the brain," says the study’s senior investigator, neuroscientist Charbel E-H Moussa, MB, PhD. Link between tumor suppressors and starvation survival suggested

CU-Boulder Professor Min Han said the research team was interested in how a common tumor suppressor gene known as Retinoblastoma 1, or Rb, behaved under conditions of starvation.
