Older breast cancer patients still get radiation despite limited benefit

The study suggests that doctors and patients may find it difficult to withhold treatment previously considered standard of care, even in the setting of high quality data demonstrating that the advantages are small. “The onus is on physicians to critically analyze data to shape our treatment recommendations for patients, weighing the potential toxicities of treatment against clinical benefit,” said Rachel Blitzblau, M.D., Ph.D., the Butler Harris Assistant Professor of Radiation Oncology at Duke University Medical Center. Ovarian cancer patients may benefit from pelvic radiotherapy

“Despite the intense therapeutic and surgical regimen typically used to treat ovarian cancer, outcomes remain poor,” said William Small Jr., MD, chair, Department of Radiation Oncology, LUHS. “This study provided encouraging preliminary results for the use of RT in women with ovarian cancer.” The study evaluated 56 patients with ovarian clear cell adenocarcinoma (CCA), an aggressive form of ovarian cancer that is more likely to be resistant to chemotherapy and to have a poorer prognosis than other forms of this disease. …

Technology developed to improve lung cancer detection, treatment

John Roeske, PhD, and colleagues presented how they developed and evaluated the technology at the recent annual meeting of the American Society for Therapeutic Radiology and Oncology. Their approach uses dual-energy imaging combined with fluoroscopy to view tumors during radiation therapy. This technology does not require an X-ray that produces both high-and low-energy images. Image Guided Radiation Therapy Is Commonly Used to Ensure Accuracy in Treating Pediatric Tumors

IGRT is the process of using frequent imaging, typically performed in the treatment room prior to radiation delivery, throughout a patient’s course of radiation therapy treatment to improve localization of the target and normal structures, which allows for more precise and accurate radiation delivery. IGRT is a common practice in both photon (traditional radiation therapy) and proton therapy to treat tumors close to sensitive structures and organs or in areas of the body prone to movement or change in shape. This study, “Practice patterns of photon and proton pediatric image guided radiation treatment: Results from an International Pediatric Research Consortium,” evaluates the use of IGRT in treatment planning for pediatric cancers in an international consortium comprised of seven institutions using either photon or proton therapy with dedicated pediatric expertise. Choosing optimal IGRT regimens that spare healthy tissue and organs is a particular concern for pediatric patients to help prevent potential late effects associated with the distribution of the radiation dose and the total radiation dose the patient receives. …

The study, using a national database of more than 100,000 women treated during the last decade, found that today nationally radiation oncologists are less likely to use radiotherapy in women older than 70 with early-stage estrogen-receptor-positive breast cancer and that when they do, treatment is appropriately less-intensive. This follows a randomized trial demonstrating low rates of recurrence in women who don’t receive radiotherapy after lumpectomy, provided they take endocrine therapy.
