Living in an area with few general surgeons may make people with appendicitis more likely to turn into ruptured appendix cases by the time they get to surgery, according to new research. “The study shows that access to surgical care, especially general surgical care, is important and low access can have real impacts that affect peoples' health,” coauthor Thomas Ricketts of the University of North Carolina at Chapel Hill told Reuters Health by email. The Affordable Care Act includes a provision for incentive payments to increase the supply of doctors in areas with shortages, but those bonuses only apply in places with too few primary care doctors. General surgeon and primary care shortage areas don't always line up, Ricketts said, and even when they do, general surgeons can't move to areas that don't already have primary care physicians to serve the community and refer patients back to them. Almost 30,000 patients with appendicitis - a quarter of whom suffered a burst appendix - were discharged from hospitals in North Carolina between 2007 and 2009, according to data from hospitals and surgery centers in the state analyzed by the researchers. There are 95,000 burst appendices per year in the U.S. according to the Agency for Healthcare Research and Quality. A 2008 study found that an appendectomy can cost between $10,000 and $18,000 more when the appendix has burst. With surgery before a rupture, typically about 20 hours or less after abdominal pain begins, patients usually recover quickly. If the appendix has burst (roughly 40 hours after symptoms begin), that could mean repeat operations and longer recovery time, according to the Merck Manual. To see whether a surgeon shortage was linked to more burst appendices in an area, the researchers compared the number of cases of appendicitis, and specifically burst appendices, with the number of general surgeons in “surgical service areas” of the state that roughly align with zip codes. Having less than 3 general surgeons per 100,000 people increased the likelihood of having a ruptured appendix by five percent, compared to areas with at least 5 surgeons, Ricketts and his colleague report in the Annals of Surgery. However, areas with the most severe shortage had a 25 percent rate of rupture, compared to 24 percent in areas with no shortage. According to Dr. Edward Livingston, who has written about using ruptured appendices as a measure of care but was not involved in the new study, that is much too small a difference to draw any conclusions. Logic dictates that ruptures would be more common in rural areas, since patients have to travel farther to get to care, but that's the opposite of what the study found if the results are reliable, said Livingston, the deputy editor for clinical content at the Journal of the American Medical Association in Chicago. There were fewer surgeons relative to the population in urban areas, but the study didn't account for the residents, physician's assistants and nurse practitioners who add to the surgical work force in large urban medical centers, Livingston said. The results of the same study with more patients from more diverse regions might not show any difference between groups at all, Livingston told Reuters Health in an email. It would make more sense to measure the delay between when symptoms begin and when the patient reaches surgery in several different areas, and see if areas with longer delays correspond to areas with fewer surgeons, in order to infer that fewer surgeons lead to delays which lead to ruptures, Livingston said. “They would have to measure the delay to prove their point,” Livingston said. “This highlights a problem in studies like this one, where conclusions are made based on assumptions about what happens without really knowing what is happening at a patient level,” he said.source : http://www.foxnews.com/health/2013/06/17/surgeon-shortage-linked-to-burst-appendices/
Fetal alcohol spectrum disorder (FASD) and autism spectrum disorder (ASD), both conditions that are neurodevelopmental in origin, may share some similar molecular vulnerabilities, according to a new rodent study published in Alcoholism: Clinical & Experimental Research. When researchers from Northwestern Medicine in Chicago, Ill., exposed pregnant rats to alcohol, they found their offspring experienced symptoms of social impairment and altered-levels of genes that have been previously linked to autism in humans. “The novel finding here is that these two disorders share molecular vulnerabilities, and if we understand those, we are closer to finding treatments,” Eva Redei, the senior author of the study and professor of psychiatry and behavioral sciences at Northwestern University Feinberg School of Medicine, said in a press release. Furthermore, study authors found that when the pregnant, alcohol-exposed rats were given low doses of the thyroid hormone thyroxin, they were able to lessen some of the effects of alcohol damage and reverse the expression of autism-related genes in offspring. Genomics and particle physics - offering different perspectives on the fundamental nature of life and the cosmos - are the two hottest areas of scientific research. Eight of the 21 most closely followed scientists in 2012 studied genes and their functions, while the single most-cited paper last year covered the hunt for the long-sought Higgs boson particle, according to a Thomson Reuters survey on Wednesday. It was the third year in a row in which genomics researchers topped the rankings, in terms of authoring the most highly cited scientific papers, underscoring the central importance of genetics in biological science and medicine. “Genomics is a perennially hot topic as we learn more about how (DNA) sequences play out in the manifestation of disease,” said Christopher King, editor of Thomson Reuters ScienceWatch, which tracks trends in research. The relevance of the work in genomics was evident this week at the American Society of Clinical Oncology (ASCO) congress in Chicago, where key advances in cancer medicine on display hinged on understanding the genetic basis of tumors. The world's “hottest” researcher, as measured by the number of citations during 2012 for papers published between 2010 and 2012, was Richard Wilson at the Washington University School of Medicine, the survey showed. Wilson's laboratory was the first to sequence the genome of a cancer patient and discover genetic signatures related the development of disease. Formation of the universe Other hot genomics researchers on the list included Eric Lander of the Broad Institute of MIT at Harvard and Kari Stefansson, the founder of Icelandic biotech company Decode Genetics, which was acquired last December by Amgen. Papers related to the search for the Higgs boson accounted for nearly one fifth of the 51 papers published in the 2012 hottest research list. The boson and its linked energy field are viewed by physicists as vital in the formation of the universe and in giving mass to matter. No single scientists working on the Higgs particle, however, were identified in the rankings because of the highly collaborative nature of the particle physics research, with some papers involving upwards of 3,000 authors. Scientists working on the ATLAS experiment at the Large Hadron Collider at CERN, the European Organisation for Nuclear Research, outside Geneva received an honorable mention as a group. Gene flaws that raise the risk of breast cancer are surprisingly common in black women with the disease, according to the first comprehensive testing in this racial group. The study found that one-fifth of these women have BRCA mutations, a problem usually associated with women of Eastern European Jewish descent but recently highlighted by the plight of Angelina Jolie. The study may help explain why black women have higher rates of breast cancer at young ages - and a worse chance of survival. Doctors say these patients should be offered genetic counseling and may want to consider more frequent screening and prevention options, which can range from hormone-blocking pills to breast removal, as Jolie chose to do. “We were surprised at our results,” said the study leader, Dr. Jane Churpek, a cancer specialist at the University of Chicago. Too few black women have been included in genetic studies in the past and most have not looked for mutations to the degree this one did, “so we just don't have a good sense” of how much risk there is, she said. Churpek gave results of the study Monday at an American Society of Clinical Oncology conference in Chicago. The researchers include Mary-Claire King, the University of Washington scientist who discovered the first breast cancer predisposition gene, BRCA1. Jolie revealed a few weeks ago that she carries a defective BRCA1 gene, giving her up to an 87 percent risk of developing breast cancer and up to a 54 percent risk for ovarian cancer. The actress's mother had breast cancer and died of ovarian cancer, and her maternal grandmother also had ovarian cancer. An aunt recently died of breast cancer. Children of someone with a BRCA mutation have a 50 percent chance of inheriting it. In the U.S., about 5 to 10 percent of breast cancers are thought to be due to bad BRCA genes. Among breast cancer patients, BRCA mutations are carried by 5 percent of whites and 12 percent of Eastern European (Ashkenazi) Jews. The rates in other groups are not as well known. The study involved 249 black breast cancer patients from Chicago area hospitals. Many had breast cancer at a young age, and half had a family history of the disease. They were given complete gene sequencing for all 18 known breast cancer risk genes rather than the usual tests that just look for a few specific mutations in BRCA genes. Gene flaws were found in 56, or 22 percent, of study participants; 46 of them involved BRCA1 or BRCA2 and the rest were less commonly mutated genes. Harmful mutations were found in 30 percent of black women with “triple-negative breast cancer” - tumors whose growth is not fueled by estrogen, progesterone or the gene that the drug Herceptin targets. Doctors have long known that these harder-to-treat cases are more common in black women. The National Cancer Institute, the Breast Cancer Research Foundation and Komen for the Cure paid for the study. It included many younger women and those with a family history of cancer, and they are known to have higher rates of gene mutations that raise risk, said Rebecca Nagy, a genetics counselor at Ohio State University and president of the National Society of Genetic Counselors. Still, “it has always stumped us” to see black families with lots of breast cancer but no mutations that can be found in ordinary testing for BRCA genes, she said. That was the situation for Alicia Cook, 44, a Chicago woman whose grandmother died of breast cancer, mother died of ovarian cancer and two sisters have had breast cancer. When she was first diagnosed with breast cancer nearly 10 years ago, a test for BRCA mutations was negative. Doctors said, “I'm sure there's something going on genetically” but they didn't have the tools to find it, Cook said. Last year, she had a recurrence and a sister who was diagnosed with the disease learned she carried a BRCA1 mutation. Cook was retested for the same mutation and found to have it.
